Policy Forum for Wales seminar – integrating health & social care
The Policy Forum for Wales is holding a seminar on Integrating health and social care - service redesign, resources and next steps for delivery.
The seminar will focus on assessing the progress of integrating health and social services in Wales, with a particular focus on the delivery of care for older people.
There will be an opportunity to discuss progress in implementing the Welsh Government's Framework for Delivering Integrated Health and Social Care for Older People with Complex Needs, introduced in April 2014 - which aims to ensure services, care and support are designed, co-ordinated and delivered effectively and meet the needs of patients.
Delegates will also assess the impact of the Intermediate Care Fund since its introduction and the next steps to ensure further collaboration between services.
