The Disclosure and Barring Service has issued a reminder that old application forms are no longer valid from today (December 1, 2014).

The DBS has been sending out copies of the new Disclosure Application Form (with the F01 number at the top) to registered bodies that use paper forms – it now warns all old forms left in stock should be destroyed to avoid confusion as from 1 December we will no longer accept applications using the old form.

Please ensure you start using the new forms and have destroyed all stock of the old ones.

• The new form can easily be identified as the new form reference number at the top right-hand side begins with F01.
• The form reference number on the old forms begins with F00.
