Fairfield is registered to provide 43 care beds for males and females over the age of 45 years.
The home supports individuals who require nursing care and assistance with social needs.
Each resident receives a person-centred care plan that also considers the holistic aspects of daily living. This includes a structured management plan designed to meet their primary needs.
Model of Care
Fairfield’s dedicated staff team is experienced and trained to work with the registered group of residents. This includes:
- Residential Care – The team is experienced in caring for individuals with mild dementia, mobility challenges, incontinence, and other age-related difficulties.
- EMI Residence – Support is provided for individuals with mild dementia, mobility issues, incontinence, and other age-related conditions. Each resident’s care is overseen by a senior carer, who administers any required medication. The home maintains a strong relationship with the District Nurse (DN) to ensure nursing needs are met when necessary.
- Nursing & Clinical Needs – Each care plan is tailored to individual requirements. Fairfield supports a wide range of general nursing needs, including primary medical conditions, COPD, Parkinson’s disease, PEG feeding, tracheotomy care, stroke recovery, insulin management, early-stage dementia, and palliative care. These services are delivered by a suitably trained and dedicated professional team.
- End-of-Life Care – The clinical team offers a tailored service that respects the end-of-life pathway. They work alongside external agencies such as Marie Curie to provide a peaceful and compassionate experience. Staff members are trained in end-of-life support, and the home ensures that visitors have full access to their loved ones during this difficult time. Resident rooms are equipped with appropriate seating for visitors and are located in a quiet area of the home, where the environment can be adjusted to meet individual wishes and needs.
- Bariatric Care – Fairfield supports residents requiring bariatric care. A dedicated bedroom has been adapted for individuals with higher body weight, featuring large doors that open into the garden to ensure full access to the home’s activities and social integration.
- Outreach Services – On an interim basis, Fairfield offers an outreach service for residents placed under an interim care bed. This service supports individuals transitioning back to their homes. Residents awaiting a suitable home care package under the Integrated Care Board (ICB) receive continued support from the staff who cared for them during their stay. This includes daily visits and collaboration with external agencies to ensure the individual’s needs are fulfilled and rehabilitation is successful.
